Getting Started
Step 1: Determine if your Program or Activity falls under the policy, "Protection of Minors on Campus."
Step 2: If your program or Activity falls within the scope of the regulation, register your program. All Covered Programs must be registered and approved by the appropriate dean or vice chancellor (or approved designee) at least 60 days prior to the initiation of the program or activity
Step 3: Contact the Office of Conferences and Institutes with questions at (336) 750-2121.
The following forms are required of all camp providers (internal or external) to submit to the Office of Conferences and Institutes prior to the start of camp of all camp participants:
- Emergency Contact Information
- Insurance and Medical Information
- Minors Involved in Activities[1]
- Participant Consent, Release, and Waiver
- Photo Release Form
- Program Expectations and Parental Permission
- Rules and Guidelines
[1] Only required if minors are to be engaged in activities as described in the form.
The following forms are required of all camp providers (internal or external) to submit to the Office of Conferences and Institutes prior to the start of camp of all staff members who will have physical or online access to minors:
- Personal Boundaries for Working with Minors
- Program Expectations
- Background Check Form for WSSU Employees
- Background Check Form for Volunteers of Internal WSSU Programs
- Background Check Form for External Groups Only
- Dietary Allergens Alert Form
